I would think on a commercial application it was a glue down installation.The carpet knap could feel like it was moving especially if it was a 26 ounce or heavier.As for as what caused the bubbles or wrinkles in a glue down installation it could be a different number of things .Using the wrong adhesive,not enough adhesive(wrong trowel size),cut to tight,not rolled with a roller,or room temperature or carpet temperature too cold for installation.All of those factors are really important especially if the carpet has a unitary backing system on it.As for the skint walls,sometimes it happens with cold weather,stiff carpet and fresh paint especially if the installers are not extra careful.
