I think I figured it out!

No idea why anyone would want to do this, but it has a cold water line that runs into the hot water outlet from the heater. Guessing it’s anti-scald as some have mentioned. Backed it all the way out and got 114f. It’ll do for now but I’m probably gonna remove it when I get some time.
